GI Alliance is launching new Population Health Management programs, including Chronic Care Management (CCM) and Principal Care Management (PCM) this month.
This program is supported by a national partner, Cosán, which specializes in comprehensive population health programs. Their mission is to offer a balanced approach of technology and personal care to help patients stay healthy between their doctor’s visits.
The CCM and PCM services will give patients extra support between their visits, with tailored, proactive care and better communication among patients, their doctors, and their care teams.
“Adding population health support to our practice ensures that patients with chronic, complex gastro conditions receive the highest level of physician-directed care they deserve,” said Dr. Paul Berggreen, chief strategy officer of GI Alliance.
The CCM/PCM team includes healthcare professionals who provide guidance on clinical care, nutrition, medication management, and overall wellness. Patients will get timely help with their treatment, including support for scheduling appointments and monitoring their health.

Where the CCM/PCM program has been implemented, patient adherence has significantly improved, resulting in fewer emergency room visits, hospital stays, and readmissions, all while enhancing patient health outcomes.
